    Medicare Part D Services
    CVS Caremark is Affinity's pharmacy benefits manager for our Medicare Part D. Members in these programs can get prescription drugs not only at CVS pharmacies but all the other chain and independent pharmacies that participate in our network.     Healthways SilverSneakers® Fitness Program:
    (For Affinity Medicare Ultimate, Affinity Medicare Passport Essentials (HMO), Affinity Medicare Passport Essentials NYC (HMO), and Affinity Medicare Passport Select (HMO) Plans Only).
    SilverSneakers® is a fun, energizing well-being program that helps active members take greater control of their health through fitness classes, physical activity, education and social interaction.     Vision Services:
    Superior Vision is a national vision benefits manager. Superior Vision’s network of optometrists provides routine vision care for our Affinity Medicare Members. Visit our search portal to find a vision provider near you, or contact Superior Vision directly at 1.800.879.6901 (TTY 711).

    Dental Services:
    DentaQuest provides dental services to our Members through their network of contracted dental providers. Visit our search portal to find a dental provider near you, or contact DentaQuest directly at 1.866.731.8004 (TTY 711).

    Chiropractic and Acupuncture Services:
